Lots of FUN for students and their buddies!  Here’s what you need to know:

Please RSVP below for an accurate count of prizes, food and goody bags for all.  Students and Buddies should RSVP to this event.  This event is open to ages 5 and up; under the age of 5 can attend with an adult guardian.  Please note that you can attend this event without a new buddy at a nominal charge.  $20 cash or check made payable to AKMA at the door.

Halloween Party

    This party is designed for our students and their new buddies to attend and have fun. Students and the public can still attend this event without a buddy - they will just need to pay a nominal charge of $20 at the door by way of cash or check. Buddies are people that are invited by our active students and/or staff, are people that are not students, not a family member of an existing student or a person who has never been to any of our AKMA buddy day events. You must be over the age of 5 to attend. If you are under the age of 5 you must be accompanied by a paying, adult guardian.

    If you do not bring a buddy by the guidelines listed above, you are more than welcome to come and have a good time! Registering on this form will help secure your spot. Just bring $20 cash or check when you come to the event drop off and we will take care of everything else!

I hereby voluntarily and knowingly assume all risks and dangers inherent and incidental to the activities of the program. I will not hold AK Martial Arts and Fitness, Inc., venue facilities, vendors, its volunteers, associates, members, agents or other attending guests liable for any injuries, personal equipment and items lost incurred during event. I acknowledge that AK Martial Arts & Fitness has full rights and ownership to said media with no compensation to me or competitor; this media can or will be used to market similar events for AK Martial Arts and Fitness, Inc. I , the undersigned parent or person having legal custody / guardianship of the minors shown above acknowledge that there is no existing medical condition, food allergy and/or other condition of importance that is not listed above in 'Other info' that will prohibit the mentioned minor below to participate in activities listed in the events entered.
