AKMA Testing - One Registration per Student
Please fill out this form once you or your child(ren) have been invited (via your Testing Invitation email confirmation) to test by the instructors. Please fill out this registration form once for each student that was invited to test. All Registrations must be received before the end of the testing month. All payments need to be made once your student has been notified they passes their test.
All Students - Important
Testing will be held in regular classes for Little Tigers and Senior Tigers. Dragons and Adults will test in regular classes as well as in Testing Sessions.

Students under the age of 18 need parent approval to test. Dragon and Tiger Etiquette must be followed at home and in the Do Jang for our younger students to be eligible to test. All students need the instructors approval via an Email Testing Invitation Letter before they can test. We are adding a new field to the approval of testing. Please see below for details. If there is any reason you do not want your student to test this cycle, please contact the school via email or call to address the issues together.

Please use this field to provide a note on what your Martial Arts Student has done since the last test to meet behavior or performance goals at home or at school. This might also include acts of kindness or community volunteering. Adults and teens please put how your training has helped you over the past cycle. PLEASE NOTE THAT WE READ THESE OUT LOUD AT THE TEST FOR EVERY STUDENT. Example: Amy is so excited to be promoted to Dragon and is focused when in class. She is a good listener and follows directions the first time. She is very responsible and works hard at school and is getting straight As. We are very proud of her.
