Rules and Regulations:
Ages 4.5 and up. This is a family and friend event so we would like to encourage everyone to play laser tag with their families or friends they have invited. However, we will allow Dragons and their friends over the age of 8 to attend without their parents or guardians in attendance. ALL registrations must be completed on-line via the registration form below. Registration below doesn't confirm your spot. It reserves your spot. We will call or email you directly to confirm if you are confirmed for attending or if you are on the waitlist. Calls or walk-ins will not be allowed. If you are not an AK Martial Arts & Fitness Student, you must be invited by one of our students to attend. Please wear comfortable clothing, no jewelery, shoes or socks will be allowed on the Do Jang Foor. AK Martial Arts & Fitness, Inc. reserves the right to refuse service to anyone that doesn't abide by Do Jang policies and etiquette.
Payment (Cash or Check Only) must be received by Wednesday, March 24th to reserve your spot(s)

ATTENTION AK MARTIAL ARTS & FITNESS STUDENTS AND FAMILIES! Bring a Buddy to this event and they will automatically receive a coupon for 2 weeks of free training at our Do Jang. AKMA Students will receive a $25 referral bonus for new student sign ups!

Acknowledgement and Agreement

I, the designated parent or person having legal custody/guardianship of the minors shown above, or for myself, give permission for these minors to participate in all activities as described in the announcement for the program event. I hereby voluntarily and knowingly assume all risks and dangers inherent and incidental to the activities of the program. I will not hold AK Martial Arts and Fitness, it's instructors, employees or other attending guests liable for any injuries incurred during the program whether caused by equipment or the acts or omissions of others excepting, damage or injury soley caused by the willful misconduct of negligence of Ak Martial Arts and Fitness or it's employees or agents. I, the undersigned parent or person having legal custody/guardianship of the minors shown above acknowledge that there is no existing medical condition, food allergy and/or other condition of importance that is not listed above in "Other Info" that will prohibit the mentioned minors above to participate in activities listed in the events announcement. I also acknowledge that all minors must be over the age of 5. Cancellation Policy

I understand that my $10 per person is payable by cash only and that it is non-refundable. If i am a no-show i will be expected to pay $10 per person to the school for costs incurred. If I decide to cancel, I understand that I will not receive a refund UNLESS AK Martial Arts and Fitness can find someone to take my spot(s), in which case, I will receive a full refund. I understand that AK Martial Arts and Fitness will require at least 6 attendees for each laser group session. If there are less than 6 attendees, AK Martial Arts and Fitness will reserve the right to cancel session(s). In the event of a canceled session by AK Martial Arts and Fitness, all said attendees will receive a full refund.
